Outpatient Rehabilitation
Phoebe’s outpatient rehabilitation program is a community-based service for people living in their homes and needing rehabilitation services in the Upper Bucks County area. Our therapists work one-on-one with each patient to set goals for achieving the best possible outcomes.
Services include Physical, Occupational, and Speech Therapy. Each patient’s therapy program will be designed to meet their individual needs for recovery as we help return them to their best possible health and well-being.
Transition to Home
At Phoebe, you can stay a little longer if you aren’t ready to go home after short-term rehabilitation. If your condition worsens in the 30 days after discharge from a skilled nursing facility, you may be reinstated for Medicare coverage.
Benefits include receiving continued therapy, nursing care 24/7, medication management, assistance with activities of daily living, three prepared meals daily, and a home-like environment.
