Connecting Hearts Zoom Virtual Visits

Phoebe Ministries is pleased to offer Connecting Hearts, a program that virtually connects residents with their loved ones both near and far. Friends and family members can fill out the Connecting Hearts form below to schedule a date and time for a face-to-face video call with a resident from one of Phoebe’s personal care or skilled nursing levels of care.

Please complete the form below to submit a request for our Connecting Hearts video call with a resident of one of our four Phoebe communities. Please also select the resident’s level of care in the community where they reside. We offer Connecting Hearts video calls for our personal care and health care center residents at all four campuses.

The person wishing to video chat with a resident must have a device with video and internet capabilities (desktop or laptop with a camera, a tablet, or smartphone) and the use of the Zoom program.

Please download the Zoom app on your computer and/or smart device before your scheduled video call.
