Phoebe offers short-term rehabilitation in Wernersville, located in picturesque Berks County. Phoebe’s short-term rehab features an onsite physician, providing our patients with exceptional therapy and services specialized for older adults. We focus on physical, psychological, emotional, and social well-being in each patient’s personalized care plan. Our experienced rehabilitation team provides physical, occupational, speech, and aquatic therapies for various orthopedic and neurologic conditions, medical and postsurgical or joint replacement rehab, and pain management, among many others.

Therapy at Phoebe Berks includes individual, one-on-one therapy and specialized treatment to each patient’s needs. Patients can access a comprehensive care team, including a Social Worker, Dietitian, Resident Assessment Nurse, Financial Counselor, Chaplain, and others.

Short-Term Rehabilitation Wing

Our short-term rehabilitation wing is designed to meet patients’ needs with a comfortable, home-like space that helps patients focus on healing. The therapy gym has been recently updated and enlarged with the latest technologies. Patients can stay in one of our 22 private rooms, with access to two dining rooms, elegant visiting areas for friends and family, and an outdoor space with a furnished patio. Wireless internet access is also available.

Outpatient Rehabilitation

Phoebe’s outpatient rehabilitation program is a community-based service for people living in their homes and needing rehabilitation services in Berks County. Our therapists work one-on-one with each patient to set goals for achieving the best possible outcomes.

Services include both Physical and Occupational Therapy. Each patient’s therapy program will be designed to meet their individual needs for recovery as we help return them to their best possible health and well-being.

Transition to Home

At Phoebe, you can stay a little longer if you aren’t ready to go home after short-term rehabilitation. If your condition worsens in the 30 days after discharge from a skilled nursing facility, you may be reinstated for Medicare coverage.

Benefits include receiving continued therapy, nursing care 24/7, medication management, assistance with activities of daily living, three prepared meals daily, and a home-like environment.
