Phoebe Ministries a not-for-profit, multi-facility care organization is seeking an experienced Service Coordinator to join us on a full time basis on our team.
Education: B.A. degree in Social Work and/or post secondary work in counseling, Experience in social work and/or providing services to the elderly.
Experience: Minimum one (1) year of social work and/or experience in assisting geriatric clients required.
Skills: Proficient reading, writing, grammar, and mathematics skills; working knowledge of supportive services. Working knowledge of hospital admission and discharge procedures; proficient interpersonal relations and communicative skills; demonstrated ability in dealing with people.
Primary responsibilities will include: Provide case management and referral services to all residents including assessment of health and psycho-social needs with the development of an individually designed case plan for services. Establishes linkages with agencies and services providers and monitors services.
