Job Reporting Relationships

  • Supervised by: Wing Manager
  • Supervises: Charge Nurses, Nursing Assistants on assigned unit

BASIC REQUIREMENTS Education: Graduate of an approved school of nursing; current Pennsylvania license to practice as a Registered Nurse/Licensed Practical Nurse.  (If Licensed Practical Nurse it is desirable to pursue licensure as a Registered Nurse) Expectations: Ability to demonstrate the clinical skills your direct reports are expected to perform

  1. Proficient reading, writing, grammar, and mathematics skills
  2. Provide excellent communication, team building, customer service,
  3. Achieve all leadership accountabilities.
  4. Knowledge of drug interactions, side effects, and incompatibilities
  5. Demonstrated management and supervisory skills
  6. Possesses thorough knowledge of current long-term care nursing practices in accordance with the Pennsylvania Department of Health long-term care licensure regulations
  7. Current C.P.R. certification (or eligibility and pursuit of certification).
  8. Nursing Clinical Leadership Development is required training for this position.

Experience: Minimum of three (3) years’ experience working as a charge nurse in a long-term care geriatric setting normally required.

Accountability: Holds self, charge nurses and nursing assistants accountable for the ownership of their behaviors, their functioning as a unified team, and, their achievement of their behavioral and functional outcomes and metrics. 

Mentor Employees: Develop relationships with direct reports through coaching. Address issues in real time; give performance/competency feedback, review behavioral/values and future development. Educate/discipline as needed.

Flexibility: Flexes schedule on RCC days to ensure communication with all shifts and oversight on all shifts as needed. Accept 24/7 ownership of the unit.

Communication: Keeps personnel informed of pertinent policies and procedures affecting the department and/or their jobs; creates an atmosphere in which upward communication from employees is encouraged. Responds to questions relating to particular requests from residents, staff personnel within acceptable time frames. Remains a visible force on the unit.

Collaboration/Teamwork: Ensures Charge nurses consistently own, develop, lead and manage their direct reports. Instills a culture of teamwork.

Stewardship: Seek out opportunities to decrease departmental expenses and waste.  

OUTCOME/METRIC RESPONSIBILITIES  Responsible for achievement of all assigned outcomes and metrics. Ability to identify areas of opportunity and drive the implementation of corrective action plans to affect change. Achievement in outcomes/metrics will result in:

  1. Improvement in assigned quality measures
  2. Decreased customer grievances on assigned unit
  3. Decreased hospital readmissions of assigned residents
  4. Increased customer satisfaction through Customer Driven Culture
  5. Compliance with documentation standards
  6. Monitor lean spaces/processes and assure compliance


  • Supervises the care given to residents, monitors guarded and critically ill residents.
  • Oversight of care planning – completion, content and implementation, attends resident care conference, evaluates needs for changes in and monitors implementation of plans of care. Communicates with resident/representative to assure their involvement in planning their care and that they are informed of changes. Evaluates restorative programs as a part of the care planning process, works with resident/representative on person centered plan and documents Restorative note. Educates staff on the units on resident’s plan of care and any changes.
  • Oversight of Antipsychotic medication use, collaboration with physicians to decrease use, attends antipsychotic meeting.
  • Assures accurate assessment and appropriate interventions, promotion of care in house if possible to prevent hospital readmissions, and promotion of Customer Driven Culture in relation to the resident.
  • Weekly measurement of wounds and communication with physicians for care, weekly review of treatments to assess effectiveness, need for a change in the plan of care
  • Follows up on incidents to assess for risk to the resident and/or facility. Monitors follow-up assessment and documentation, assesses need for interventions for the resident, and changes processes on the unit as needed to mitigate risk
  • Communicates with pharmacy personnel with regard to medications
  • Insures an atmosphere which allows for the privacy, dignity, and well-being of all residents in a safe, secure environment.
  •  Insures resident safety by maintaining awareness of individual responsibilities to follow standard/universal precautions and adhere to the established Fire Safety/Disaster plan.
  •  Insures the confidentiality of all resident- and/or employee-related information.
  •  Monitors resident acuity, reports changes to the Wing Manager and directs staff assignments on the unit accordingly. 


  • First line of communication with residents/families/representatives to improve care/customer service
  • Collaborates with hospitals and insurance carriers to maintain preferred status.
  • Collaborates with laboratory/radiology services to provide necessary information
  • Collaborates with the IDT to decrease risk, improve care on the unit
  • Communicates daily and is a resource for input from staff daily. Communicates needs, trends, complaints, etc., of staff to Wing Manager.
  • Works with the Nursing management in evaluating and creating plans for improving the nursing service to residents and improving employee relations.  


  1. Performs tasks which are supportive in nature to the essential functions of the position, but may be altered or redesigned depending upon individual circumstances.
  2. Completes all mandatory in-service training programs and attends required meetings.

 Occupational Health Requirements

  1. Ability to lift 50 pounds.
  2. Proper use of Universal Precautions
  3. Ability to lift, bend, push, and pull.
