Applying for long term care can be a stressful and time-consuming process. Phoebe has made it easier for you with our new pre-qualification forms for personal care or independent living at any one of our communities that offer those services.

All you have to do is fill out the financial information on the back of the form and send it to the address listed on the front. Our admissions team will take care of the rest!

By filling out the form at home, you can take your time and double check all of your information without having to worry that you’re forgetting something. And don’t worry—if you need help or have any questions along the way, we’re here for you. Just call the number listed on the front of the form.

Once you send your form in, an admissions counselor or sales staff member will review the information you provided. You can speak to our staff on the phone, or make an appointment and take a tour of the facility you’re applying for.
