Phoebe Ministries Announces Staff Change

Allentown, PAPhoebe Ministries is pleased to announce the following change in staffing:

Tracy Polacheck has been appointed to the position of Executive Director of Pathstones by Phoebe. In this role, she is
responsible for the overall operations of the Continuing
Care at Home program and oversees the development and implementation of comprehensive programs and service offerings to help members stay well and maintain their highest level of independence.

Pathstones by Phoebe is a groundbreaking endeavor by Phoebe Ministries and offers a personalized plan for care coordination and the services of a Continuing Care Retirement Community (CCRC) in the comfort and security of a member’s home. Like a traditional CCRC, Pathstones by Phoebe members pay a one-time entrance fee and monthly membership fees. Fees depend on the applicant’s age and the plan selected. They cover the cost of future services, but are generally lower than the rates at a traditional CCRC. Monthly fees will not go up if a new level of care is needed.

Most recently, Polacheck served as Senior Executive Director at Keystone Villa Communities in Ephrata where she was responsible for overseeing the operations of Independent, Personal, and Memory Care.

With nearly 20 years’ experience, she has spent the majority of her career opening and managing a wide spectrum of retirement communities in Berks and Lancaster Counties. She is a Licensed Administrator for both Personal Care and Assisted Living.

A resident of the Reading area, Polacheck graduated from Penn State University with a Bachelor’s Degree in Kinesiology with a concentration in gerontology.
