Let us help you return home. Phoebe Wyncote offers physical therapy, occupational therapy and speech therapy, provided by professional therapists. We will work with you to meet your goals.
Our nurses and physicians will treat your medical needs and help you prepare to return to the community. Our social worker will meet with you regularly to determine your discharge planning needs to return home. Whether it be to set up home health services at your home, ordering medical equipment for you, our social worker will help you transition to a level of care where you can be most independent.
If you do not decide to return home, you may consider moving to Phoebe Wyncote’s personal care apartments or private rooms, or to one of our independent living apartments.
